- 2. General provisions
These Terms set out the rules and conditions for using the Service.
- The purpose of the Service is the provision by MEDIQUS of Services enabling Clients to contact Jakub Bukowczan MD PhD directly.
- In these Terms we use concepts to which we have given a specific meaning. All definitions can be found at the beginning of these Terms.

- These Terms are available on the website www.mpacz.pl in a manner that allows you to obtain, reproduce and record their content.
3. How the Service works
- The contract is concluded:
- at the moment you establish a voice connection to the premium-rate number; the contract ends when the voice connection to that number is terminated.
Premium-rate voice calls are charged every 60 seconds in advance for each minute commenced.
- The time for responding to your premium-rate SMS message is seven working days and may be extended, of which you will be notified.
- The premium-rate telephone consultation service is available on working days between 9:45 and 10:45 and between 22:00 and 23:00.
- In order to use the Services you must have a device enabling access to the public telephone network with the premium-rate number barring disabled.
6. Reklamacje
- The Service operates only for Polish telephone numbers, unless these Terms and the annexes provide otherwise.
- Detailed technical rules and the manner of using the Services can be found in Annex 2 to these Terms.
- 4. Payments
- Payment is made through your telecommunications operator providing the public network to your Device.
Prices are given in Polish zloty and include all components, including VAT.
- 5. Right of withdrawal
- By establishing a connection to the premium-rate number you consent, before MEDIQUS begins performance, to the loss of your right to withdraw from the contract once it has been performed.
- By establishing a connection to the premium-rate number you request that MEDIQUS perform the contract before the expiry of the withdrawal period.
You cannot withdraw from the contract once it has been performed by MEDIQUS.
- 6. Complaints
- Complaints arising from an infringement of your rights guaranteed by law or under these Terms may be submitted to recepcja@mpacz.pl.
- In your complaint you should provide your name, correspondence address, and the nature and description of the matter.
We undertake to consider every complaint within fourteen days, and where this is not possible we will inform you when the complaint will be considered.
- You will be informed of the outcome of the complaint procedure by e-mail, unless you have not consented to this form of contact.
- 7. Out-of-court complaint and redress procedures
- If you have submitted a complaint to us but for some reason we have not upheld it, you may apply for mediation or a decision to one of the permanent consumer arbitration courts operating at the Trade Inspection Authority.

- We also inform you of the existence of the European Online Dispute Resolution platform and the possibility of using it to resolve disputes:

- You may also obtain free assistance in resolving a dispute between us from the district or municipal consumer ombudsman.
- 8. Final provisions
Any disputes arising between us and you shall be resolved by the courts having jurisdiction in accordance with the relevant provisions of law.
- In matters not governed by these Terms, the provisions of the Civil Code, the Act on the provision of services by electronic means and the Consumer Rights Act shall apply.
